Flu Vaccine Clinic for Students – October 21, 2014

MSAD35 Flu Vaccination Clinic

The Maine Center for Disease Control and Prevention recommends a yearly flu vaccine for all children ages 2 to 18 years old as the first and most important step in protecting against flu. This year we will hold one clinic for all students in MSAD 35 after school hours to provide an opportunity for students to be vaccinated. This clinic will be held at Marshwood High School on Tuesday, October 21, 2014 from 3 to 7 PM in the gymnasium. There will be no clinics held during the school day.

Please review the vaccine information sheets provided by the CDC below and print the Health Screen & Permission Form. You can fill out the form and bring it with your child to the clinic on October 21. Permission forms will also be available at the clinic if you are unable to print the form at home. We will have injectable vaccine and a supply of nasal mist vaccine.

Health Screen Permission Form – Permission forms will be available at the clinic if you are unable to print and fill them out ahead of time.
