The following documents will be completed when approved to substitute in the district.
Payroll Documents – W-4 Federal Form, W-4ME State Form, Form I-9 Employment Eligibility Verification (proof of proper identification in person is required for form), Direct Deposit Form and District Emergency Contact Form.
Maine Public Employees Retirement System – Optional Membership Application as a substitute and Statement Concerning Your Employment in a Job Not Covered by Social Security.
District Workers’ Compensation Health Care Providers Notice
403B – TSA Consulting Group, Inc. optional payroll deductions for retirement savings.
